Identification Document Submission Form

In order to verify ownership, we request that you send us a copy of a piece of official identification.

Please enter your Square Enix ID, email address and ticket number and click the "Confirm" button to proceed to the confirmation page. You will then be asked to attach an image of one of the following identification documents:

Passport (preferred)

Driver's License

Birth Certificate

National ID Card

If you have been requested to send any other documents such as proof of purchase, please refer to the email received from the Support Centre team for further instructions.

Information will be sent encrypted. Copies of the documents sent will be kept only for the purpose of identity verification and will be securely destroyed within 90 days of the completion of identity verification.
